A waiter was observed dropping off dirty dishes and then without washing his hands: started filling people's cups and touching their straws.

Several foods were being held at the wrong temperature and another waiter was observed placing lemons into drinks with his bare hands.

Chad's Steakhouse on North Swan failed its inspection and is operating on a provisional license.
