Investigators determined driver error led to last week's crash that claimed four members of a Glendale church.
The Yavapai County Sheriff's Office says a 16-year-old boy was behind the wheel of an SUV when it veered off a mountain road and plunged 100 feet.
A church pastor said the victims were heading to a men's retreat near Crown King north of Phoenix.
The other victims of the crash include 47-year-old Kevin Waycaster, 67-year-old Garry Schneider, and Robert "Bob" Fisher, a Phoenix man in his 70s.
The driver, 16-year-old Micah Waycaster, of Glendale also died.
